If you highlight a variable you get a distinctly different context menu when executing 'refactor this' then you do if the cursor is on the variable but not highlighted!
Most annoyingly is the missing 'rename' option.

I have seen a similar problem with VS2010 Beta 2. The rename option is really useful.
